To reserve a pavilion at the Chazy Rec Park, please complete and return this form. You must be a Town of Chazy resident. You will receive an email or phone call confirming your reservation.

Dog Licenses

According to New York State law, all dogs must be licensed at FOUR months of age; A current Rabies Certificate and proof of spay or neuter, if applicable, are required. If your dog is microchipped, please send a copy of that paperwork as well.

Vital Records

The Town Clerk is appointed Registrar of Vital Statistics for the New York State Department of Health. Registrars are NOT allowed to release any information prior to receiving an application, including whether or not a record is on file, even if the person who the record pertains to is making the request.

Marriage Licenses

Marriage License Worksheet - Please make an appointment. Completed worksheet must be returned to the Town Clerk at least 24 hours before your appointment.

Marriage License Requirements:

1. Proof of Age - Original Live Birth Certificate, Certified Copy, or Certified Transcript of Birth (NOT Certificate of Record or Certificate of Registration which only states that there is a birth record on file)
2. Photo Identification - Valid Driver's License or Passport
3. Previous Marriages - Certified Copy of the Decree of Divorce or a Certificate of Dissolution of Marriage
4. $40.00 fee payable to the Town of Chazy
