According to New York State law, all dogs must be licensed at FOUR months of age; A current Rabies Certificate and proof of spay or neuter, if applicable, are required. If your dog is microchipped, please send a copy of that paperwork as well.

1. Proof of Age - Original Live Birth Certificate, Certified Copy, or Certified Transcript of Birth (NOT Certificate of Record or Certificate of Registration which only states that there is a birth record on file)
   Birth record must show birth name, birth date, birth place, and parents' names
2. Photo Identification - Valid Driver's License or Passport
3. Previous Marriages - Certified Copy of the Decree of Divorce or a Certificate of Dissolution of Marriage
4. $40.00 fee payable to the Town of Chazy
